As of the fourth quarter of 2023, Cottonwood Creek Skilled Nursing and Therapy was owned by Bridges Esop, Inc., according to Centers for Medicare and Medicaid Services (CMS). The nursing home, located in Grady County, has been owned by Bridges Esop, Inc. since Dec. 31, 2020.

But when you need that type of care, there are limited options.”
